• Additional Info: We are a rapidly growing, entrepreneurial not-for-profit Federally Qualified Health Center and social service agency that serves the homeless and unstably housed people in New York City.DETAILS:- Reaches out and engages clients with appropriate concern for their behavioral health care needs- Coordinates medical care with PCP for chronic and acute non-behavioral health conditions- Advises and counsels clients about their physical and mental health; coordinates care with case management and health education as clinically indicated- Assists with the identification, counseling, and care coordination, with case management and health education, of mental health and substance abuse problems- Works in a non-traditional environment; e.g., the non-clinic facility where homeless people congregate- Provider will be servicing the men who live in the shelter initially, and patient panel will expand as role expands- EMR = eClinicalworks
