Mental Health Chart Analyst

3850 Crenshaw Boulevard Los Angeles, CA 90008, USA Responsibilities Functions as therapist for clients in the SCHARP FSP 2 Functions as QMHP for assigned clients; ensuring Adult Initial Assessment, Treatment Plans, Tri-Annual Assessments and other required updates are clinically relevant to the individual client as well as reviewed, completed and signed in a timely manner as specified by Los Angeles County Department of Mental Health standards. On an ongoing basis, complete all required DMH clinical services documentation, Outcomes Measures and other paperwork assigned within prescribed time frames Works with SCHARP program treatment team to coordinate services and monitor client treatment plans and treatment goals. Prepare for weekly clinical staff meetings and supervision by having current information on the status of all assigned clients and program assignments. Review 85 charts per month (including new cases enrolled, cases discharged, and random selection of active cases to meet monthly quota) utilizing the Case Record Review tool to ensure documentation meets SCHARP standard. Email assigned staff and supervisor regarding missing items and follow up to ensure needed items are fixed and re-submitted. Provide monthly report to Program Director of chart review including average scores, areas of deficiency, and areas further training may be needed. Other duties as assigned.
