RN Care Manager - Child Center - Long Term Care

Job Description RN Care Manager at Providence Child Center, Long Term Care, Portland, Oregon. Work hours: Monday–Friday, 7 am–4 pm. We provide compassionate, resident‑centered care to a diverse population of medically fragile individuals, ranging from children with complex developmental and neurological conditions to adults with chronic illnesses and disabilities. Some residents require long‑term ventilator support, tracheostomy care, and assistance with activities of daily living. Our team is known for exceptional continuity of care and interdisciplinary collaboration with respiratory therapy, rehab, and social work. Responsibilities Accountable for quality of care for children, according to individualized needs and the nursing process, in accordance with the Nurse Practice Act. Manage nursing care of assigned residents, ensuring maximum independence and self‑direction. Coordinate care provision with physicians and other interdisciplinary team members. Ensure resident care plans are developed, implemented, and documented to meet individual care needs. Participate in professional development activities. Required Qualifications Bachelor's degree in Nursing. Oregon Registered Nurse license upon hire. Background check as required by DHS (every 2 years). Minimum 3 years of nursing experience, 2 of which in care of medically fragile children and adults. Recent relevant experience. Preferred Qualifications Master's degree in Nursing. 1 or more years of care/case management experience. Leadership experience. Benefits Free, convenient, ample parking.
