Full-Time Home Care Physical Therapist (Pt) - Motion Restore Therapy

FOX Rehabilitation is seeking a compassionate Physical Therapist to provide evidence‑based geriatric rehabilitation services. In this clinician‑owned practice, you will evaluate and treat older adults in a variety of settings, develop individualized treatment plans, and focus on functional outcomes that enhance quality of life. Collaborate with an interdisciplinary team, document care accurately, and educate patients, families, and caregivers. Responsibilities Provide outpatient rehabilitation services in an in‑home setting for the older adult population. The in‑home setting could be a patient’s private residence or in a patient’s room in an independent living facility. Evaluate and treat patients, develop and deliver individualized physical therapy programs to facilitate rehabilitation. Identify the need for and make referrals to other disciplines to address the comprehensive needs of patients. Implement individualized therapy programs designed to restore, reinforce, and enhance performance. Instruct, educate, and train patients and caregivers in skills essential for promoting independence and productivity. Document care accurately, adhere to department and patient treatment schedule, and complete timely clinical and billing documentation. Collaborate with interdisciplinary teams (PT, OT, SLP, and fitness specialists) to select appropriate therapeutic regimens. Adapt program and/or plan of care according to patient needs. Promote a safe environment and enforce safety regulations. Order supplies and equipment as necessary and report any required repairs. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy; or Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience. Valid driver’s license or reliable, independent transportation.
