Care Manager I

Care Manager I (CMI) The Care Manager I is responsible for the assessment of resident psychosocial needs and coordination of support services for low‑income older adults. The CMI also plans for and monitors services and interventions ensuring provision of quality care. Responsibilities Conduct comprehensive assessments and ongoing re‑assessments of clients, including psychosocial, physical, mental health, environmental and spiritual needs. Write comprehensive assessment reports. Develop and initiate client‑centered Community Living Plans in line with program guidelines. Conduct home visits, acute hospital and skilled nursing facility visits, and escort clients to medical and other appointments as clinically indicated. Identify, arrange for and monitor appropriate community services based on knowledge of Medicare, Medi‑Cal and other entitlement programs. Establish and maintain a care‑management relationship with clients and their significant others, offering respect, dignity and support. Provide crisis intervention, advocacy, problem solving and therapeutic interventions. Meet with clients at least monthly, and more often as clinically indicated. Review and modify care plans on an ongoing basis. Document case‑management activity via progress notes within 24‑48 hours, adding new problems to the care plan as needed. Maintain required paperwork and charting to allow for continuity of care. Evaluate client purchase of service needs and follow up to ensure timely provision of services. Educate clients and significant others about available resources. Maintain open and effective communication with community providers, including physicians and other health care and social service workers. Provide necessary information on significant aspects of client care while maintaining confidentiality. Monitor the quantity and quality of services provided by other providers. Collaborate with the team to evaluate clients’ ability to remain safely at home; coordinate placement as appropriate per Community Living Fund guidelines. Assist in terminating clients when appropriate, documenting the process. Participate in data collection and continuous quality improvement efforts. Attend team and program meetings, activities and problem‑solving endeavors, contributing to open lines of communication. Utilize supervision appropriately, providing updates on caseload activity. Incorporate ethical and legal standards of the National Association of Social Workers into all interactions. Apply regulatory and procedural requirements of Community Living Fund and Institute on Aging policies. Attend continuing education classes or in‑service training to enhance knowledge related to case management, gerontology, and related areas. Perform other reasonably related responsibilities as assigned. Education B.A. or B.S. in Social Work or a related field is required. Background & Experience One year of experience working with disabled adults and/or older adults. Understanding of medical and psychosocial problems of functionally impaired adults and older adults. Experience with individuals diagnosed with mental or behavioral health conditions and substance abuse disorders is highly desired. Exceptional communication and presentation skills related to clients and their support systems. Demonstrated case‑management skills and experience within the community health care delivery system. Detail‑oriented with strong problem‑solving skills and ability to prioritize multiple tasks. Computer literacy required. Bilingual in English/Spanish preferred. Compensation Annual salary range: $75,000 to $78,000. Compensation may vary based on experience, education, and other factors.
