Requirements
- 2 years of experience required.
- Travel experience required.
- Current compact license must be in hand at submission.
- BLS certification required.
- Experience with Epic charting required.
- Must have recent work history within the last 2 years.

Roles & Responsibilities
- Coordinate patient care and develop individual treatment plans.
- Facilitate family meetings to discuss patient needs and care options.
- Arrange durable medical equipment (DME), skilled nursing facility (SNF) placement, and ambulance transport as needed.
- Conduct needs assessments and manage prior authorizations efficiently.
- Perform utilization reviews and document in accordance with relevant guidelines.

Compliance / Notes
- Orientation consists of 40 hours.
- Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).
- Include all requested time off at the time of submission.
- Time may be refused if traveler does not float to sister facilities as needed.
- Provide Social Security Number and Date of Birth during compliance.
