Responsibilities
- Provide direct nursing care to residents in a long-term care setting, including monitoring vitals, administering medications and treatments per physician orders, and assisting with activities of daily living.
- Perform ongoing assessments and document changes in resident conditions accurately in the charting system.
- Collaborate with the interdisciplinary team (RN, physicians, therapists, social workers) to implement and adjust individualized care plans.
- Recognize and report acute changes in residents’ conditions promptly to the supervising nurse and physician as needed.
- Ensure adherence to infection control, safety, and privacy policies; maintain clean, safe, and organized units.
- Respond to call bells, participate in rounds, and provide compassionate, resident-centered care.

Requirements
- Active Licensed Practical Nurse license in Vermont
- CPR/BLS certification
- Ability to work Evening and/or Overnight shifts
- Strong communication and documentation skills
- Willingness to participate in a travel assignment in a long-term care setting
- Ability to maintain professionalism and teamwork in a dynamic care environment
