Responsibilities
- Provide direct, patient-centered care under supervision of a registered nurse
- Monitor vital signs, patient status, and changes in condition; document findings promptly
- Administer prescribed medications and treatments per care plans and facility policies
- Assist with activities of daily living and support mobility and daily routines
- Maintain accurate documentation in patient records and communicate changes to the care team

Requirements
- Active Licensed Practical Nurse license
- Experience in long-term care or skilled nursing settings preferred
- Ability to work 40 hours per week, with Day and/or Evening shifts
- Strong communication and teamwork skills
- Willingness to work in a travel assignment in the South Burlington, Vermont area
