Job Summary

The High Needs Care Coordinator is responsible for the comprehensive coordination of behavioral health services for children with complex psychosocial needs. This position provides individual, family, and group services to children, youth, and families, and facilitates Child & Family Teams to engage members and their families in recovery-oriented treatment planning. The Care Coordinator will manage an assigned caseload, delivering services both individually and as part of a team. Responsibilities include maintaining accurate and timely clinical service records, preparing and providing reports, and submitting data promptly. The position requires strong organizational skills, the ability to work in a professional team environment, and flexibility for variable schedules, including evening and weekend hours.

Essential Duties

• Demonstrates ability to maintain confidential/sensitive information.
• Engages members and their families or legal guardians in service planning to meet the needs of children enrolled in the behavioral health system.
• Provides individual, family, and group services under the direction of a qualified Behavioral Health Professional or Technician.
• Provides intake and completion of a Core Assessment.
• Identifies needs with families and completes service plan with member and family/guardian.
• Facilitates Child & Family Teams to review service needs, treatment planning progress, goal attainment, and utilization of services and natural supports.
• Coordinates a comprehensive array of services as well as the accompanying referral and administrative requirements for service delivery.
• Provides services related to the acuity of the client's needs.
• Provides positive, pro-active communication to other system partners related to care coordination needs.
• Provides complete documentation that accurately reflects services delivered in accordance with regulatory, contractual, and agency requirements.
• Assists with Case Management functions with a caseload of 20 and meets program productivity standards.
• Provides effective crisis management and maintains necessary documentation of service delivery.
• Conducts educational activities that promote client understanding of treatment issues.
• Interfaces with DCS, the courts and other partners regarding child well-being, family stability and case planning.

• Must be at least 21 years of age (licensing requirement).
• Bachelor's degree in the field of social work, psychology, or related subject area.
• HS Diploma may be considered with relevant years of behavioral health experience that comply with Office of Behavioral Health Services staff qualification requirements.
• CPR/ First Aid certification required.
• Must be able to provide own transportation.
• Must be available to work after hours, weekends, and on-call on a rotating basis.
• Must be able to meet training and agency requirements for the position.

• Must possess or be able to qualify for and maintain a valid Arizona department of public safety level one fingerprint clearance card.
• Must possess and maintain a valid Arizona driver's license and be insurable under the agency's automobile policy.
• Bilingual in Spanish preferred, but not required.
