Work Environment

Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing medical histories and performing necessary assessments to determine the appropriate treatment plans.
• Implement and monitor treatment protocols tailored to each patient's needs, ensuring effective outcomes based on ongoing evaluations and the latest evidence-based practices.
• Maintain accurate and detailed documentation of patient encounters, including initial evaluations, progress notes, and discharge summaries, in compliance with regulatory standards.
• Communicate regularly with interdisciplinary colleagues to discuss patient progress and necessary adjustments to treatment strategies.
• Complete discharge planning and provide necessary follow-up care instructions to patients and their families.
• Participate in facility meetings and training sessions, contributing your insights and expertise to enhance team collaboration and service delivery.
• Uphold professionalism by maintaining open communication with patients, their families, and third-party payors, fostering a respectful and supportive environment.

Qualifications
