Part-Time Palliative Care Nurse Manager

Centrix Health Resources is looking for a dedicated part-time Nurse Care Manager to deliver palliative care for patients at home in Arizona. You will coordinate home visits, document patient assessments and ensure compliance while advocating for patients' needs. The ideal candidate is a licensed RN or LPN with at least 3 years of experience. Strong clinical competency and effective communication are essential.
