Learn more at Responsibilities Coordinate activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker. Assist in developing and implementing discharge plans, case management referrals, authorizations, and Medicaid state submission forms. Maintain and coordinate forms for on‑site reviewer presentation and file them accordingly. Facilitate discharge planning by interviewing patients/families, gathering data, and developing a patient‑focused discharge plan. Required Qualifications Knowledge of discharge planning, utilization management, and case management terminology in both managed and non‑managed care environments. Basic to intermediate computer skills. High School Diploma mandatory. Preferred Qualifications Current Basic Life Support (BCLS) certificate required upon hire and to be maintained. Associate degree or LVN license preferred. Experience in a hospital setting preferred. Estimated wages for this role range from $24.50 to $38.60 per hour, with final compensation determined at hiring based on skillset, experience, education, credentials, and licensure.
