The posted compensation range of $36.51 - $59.00 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Job Summary and Responsibilities

This is a PRN position and will be primarily for service area of Teller County

As a Home Health RN, you will:

• Be a clinical leader. Apply your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care.
• Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being.
• Collaborate and coordinate. Work seamlessly with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care.
• Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.
• Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
• Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery.

Job Requirements

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience. Home Health experience required.
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
