Responsibilities
- Perform initial and ongoing assessments of residents to determine rehabilitation needs and establish measurable goals.
- Develop and implement individualized physical therapy plans of care, delivering evidence-based interventions and modalities as appropriate.
- Provide direct therapeutic sessions, monitor progress, and adjust treatment plans to optimize function and safety.
- Document evaluations, progress notes, treatment plans, and discharge planning in accordance with regulatory and facility requirements.
- Collaborate with interdisciplinary team members (nursing, OT, speech therapy, physicians) to coordinate care and support resident outcomes.
- Contribute to discharge planning and referrals to community resources or follow-up services as needed.

Requirements
- Current Physical Therapist license in New Mexico (or eligibility/credentialing as applicable)
- Doctor of Physical Therapy (DPT) or equivalent PT degree from an accredited program
- Experience in geriatric or skilled nursing rehabilitation preferred
- Ability to work effectively in a long-term care setting and within a multidisciplinary team
- Strong clinical documentation and communication skills
- Availability to commit to a 13-week travel assignment starting 08/31/2026
