Job Details: Job Location: Bogart - Bogart, GA 30622, Position Type: Full Time, Providence Care has merged with Your Health, and we’re expanding our team. We’re currently hiring for a Visiting Nurse to provide mobile primary care in the Lake Hartwell, Athens, Elberton areas. This is a full-time opportunity with flexibility. Our mobile approach allows you to provide care directly in patients’ homes and communities rather than being tied to a traditional office or hospital setting.

Responsibilities
- Looking for someone with a Hospice heart and passion for providing end of life care
- Provide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDG
- Responsible for overseeing patient plan of care with the collaborating physician
- Provide supervisory visits and oversight for LPN and CNAs
- Provide education to patients, families, and facility staff
- Provide oversight to Aides
Qualifications
- Must have a RN license unincumbered in the state you are applying
- Reliable transportation and valid driver's license, clean MVR
- Experience in primary care, home health, or hospice ispreferred.
