Meals on Wheels of Metro Tulsa delivers nutritious meals and caring interactions to elderly and disabled homebound individuals across several communities in the Tulsa area, including Broken Arrow, Owasso, Jenks, Glenpool, Bixby, and Sand Springs. Role Description

The Client Care Coordinator plays a key role in our More Than a Meal program, serving as a lifeline for seniors and individuals with disabilities who need support to stay healthy and independent at home.

This role is full-time (40 hours per week) and combines care coordination, resource navigation, client advocacy, and relationship building. You will assess needs, develop Service Plans, address social determinants of health, connect people to community resources, and provide meaningful support that truly changes lives.

Hours of operation: 8:00 - 4:00 M-F

Rate of Pay: $20/hour

Qualifications

• Nonprofit business experience is preferred.
• Bachelor’s degree preferred.
• Minimum two years experience in care coordination and or case management.
• Minimum two years experience working with seniors or individuals with disabilities.

• Excellent oral and written communication skills.
• Ability to interact professionally, compassionately, and respectfully with diverse populations.
• Strong technology skills and proficiency in MS Office and Office 365.
• Goal oriented, mission focused, and highly organized.
• Experience training or supervising others is a plus.
