Position Overview


Seeking a Primary Care Nurse Practitioner (NP) to provide comprehensive, community-based healthcare services to older adults through a Program of All-Inclusive Care for the Elderly (PACE).


The provider will work under the direction of the Medical Director and collaborate with physicians and an interdisciplinary team (IDT) to assess patients, develop individualized care plans, coordinate services, and educate patients and their families.


The program emphasizes holistic, patient-centered care, smaller patient caseloads, and long-term relationships that help seniors remain in their homes and communities.

Requirements
