The stability of one of Minnesota's most established home health organizations.What You'll Do Manage a home health caseload throughout Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall. Complete comprehensive OASIS assessments and individualized care plans. Deliver skilled nursing care and patient education. Collaborate closely with physicians and interdisciplinary team members. Help patients recover, remain independent, and avoid unnecessary hospitalizations. Maintain accurate clinical documentation.What We're Looking For Active Minnesota Registered Nurse (RN) license. 3+ years of RN experience (home health experience preferred). Strong assessment and critical thinking skills. Excellent communication and organizational abilities.
