Palliative Care Coordinator

Roze Room is a leading provider of Hospice and Palliative Care, celebrating over 25 years of service to Southern California communities Seeking a Full Time Palliative Care Coordinator In-Office Position Pasadena, CA Monday - Friday 9am-6pm Salary: $21.00 - $25.00/hr, based on experience. Must be computer savvy, have excellent customer service skills, Palliative Care/Hospice experience preferred. Spanish speaking preferred. What We're Looking For
- Strong computer and customer service skills
- Excellent communication and organizational abilities
- Hospice, home health, or palliative care experience preferred
- Ability to multitask in a fast-paced healthcare environment
- Professional, compassionate, and team-oriented attitude
- Valid driver's license and reliable transportation
- Coordinate palliative care referrals and admissions
- Obtain insurance authorizations and re-authorizations
- Schedule patient visits and coordinate care services
- Communicate with physicians, nurses, social workers, and care teams
- Review documentation for accuracy and timely submission
- Support patient and family needs while ensuring quality care delivery
- Maintain accurate patient records and reports
