Job Description
Home Health Physical Therapist

We are seeking a skilled and dedicated Home Health Physical Therapist to join our team. The ideal candidate will provide high-quality physical therapy services in a home health setting, assisting patients with recovery, rehabilitation, and overall mobility enhancement.

Job Details

Unit Type: PT (Physical Therapy)
Shift Length: 8 hours
Guaranteed Hours: 40 hours per week
Years of Experience: 2 years required
Shift Notes: Days, Monday-Friday, 8:00 AM - 5:00 PM
Travel: 25% travel required

Responsibilities

Assess patients' physical needs and develop individualized treatment plans.
Implement and supervise therapeutic exercises and treatments to improve mobility and function.
Document patient progress and adjust treatment plans as necessary.
Provide education and support to patients and their families regarding at-home care and rehabilitation exercises.
Collaborate with other healthcare professionals to ensure comprehensive patient care.

Qualifications

Required Certifications: BLS (AHA or ARC) Certification
License: Valid Physical Therapist License in California required
Experience: Minimum of 2 years in physical therapy
