Job Overview Title: Case Manager (I-III) Division: Project 180 – Rapid Diversion Program (RDP) Pay Scale: $25.24–$30.62 per hour Location: Antelope Valley Courthouse The Organization SSG-Project 180 turns lives around through innovative, wrap‑around services that keep people out of institutions and in their communities. Project 180 is a leader in forensic treatment, offering high‑quality, specialized programs that serve thousands of justice‑involved individuals. Our programs span criminal justice intercepts and include pre‑arrest interventions, court‑based diversions, jail in‑reach, and intensive post‑release services. Project 180 operates in partnership with county agencies including the Department of Mental Health (DMH), the Department of Health Services (DHS), and the Justice, Care and Opportunities Department (JCOD). The Program The Rapid Diversion Program (RDP) is a county‑operated initiative that diverts criminal defendants out of the legal system at the early stages of court. The program provides in‑court mental health evaluation, linkage to treatment services, and case management support. Participants’ criminal cases are dismissed upon successful completion. The Position Under the Program Supervisor, the RDP Case Manager provides wrap‑around, field‑based case management and rehabilitative services to a caseload of ~20 diversion clients. The role involves fostering collaborative relationships with external treatment providers and court staff, actively engaging clients, supporting recovery goals, and enhancing utilization of community resources. Title (I–III) is determined by years of experience. Essential Functions Provide Wrap‑Around, Field‑Based Services to Support Recovery Goals Actively engage clients through rapport, consistent contact, and motivational interviewing; provide outreach as needed, meeting clients where they are. Deliver services aligned with ethical standards, harm reduction, strength‑based, trauma‑informed care, focusing on hope, empowerment, self‑responsibility, and meaningful roles. Develop and maintain knowledge of community resources; conduct needs assessments; link clients; monitor participation; solve barriers; accompany clients to appointments when appropriate. Teach prosocial and recovery skills (communication, problem‑solving, anger management) for long‑term community stability. Engage family, court, or other collateral parties, provide mental‑health recovery education, and advocate for clients. Conduct custody pick‑ups from LA County jails. Support Clients in Adhering to Court Requirements Help new clients understand court processes, conduct, and diversion responsibilities. Assist clients in attending scheduled court appearances, providing transportation and accompaniment as needed. Procure written progress reports from community providers and submit them to court on time. Build strong relationships with RDP court staff to ensure services align with client responsibilities. Documentation and Reporting Maintain comprehensive case notes in the electronic health record per program guidelines. Participate in program data collection per guidelines. Fulfill Baseline Project 180 Staff Expectations Regular attendance; holiday or weekend work may be required. Maintain agency mission, values, policies, procedures, and principles. Perform other duties as needed. Maintain active license and/or registrations as applicable (e.g., BBS, CCAPP). Minimum Qualifications Required Experience working with culturally diverse populations. Valid CA driver’s license; proof of insurance and own vehicle; ability to transport clients in personal vehicle as needed. Spanish speaking. Must not be on any type of legal supervision (probation or parole). Verification of employment eligibility and background check. Basic computer skills. Case Manager I: Case Manager II: 2 years experience. Case Manager III: 3+ years experience. Preferred Previous case management experience. Lived experience with homelessness, mental health/substance use recovery, or the criminal justice system. Experience working with forensic and/or dual‑diagnosed populations. Supervisory Responsibilities None Environmental Conditions (Working Conditions) Job duties are performed both in office and in field settings, including the assigned courthouse. Physical Requirements The employee is expected to move throughout the day, walk, stand, climb stairs, or use a mobility device. The employee is expected to drive, carry a maximum of 20 lbs, listen, and speak to complete job requirements. Mental Requirements The employee must be able to perform complex, varied tasks such as synthesizing information and making decisions without immediate supervision; handle routine distractions, interruptions, and changing priorities; interact professionally in high‑stress situations; create and follow plans; meet deadlines. #J-18808-Ljbffr
