Responsibilities Collaborate to provide management and delivery of patient care plans. Support patients in Naperville, Plainfield, and surrounding areas. Meet critical deadlines for charting. Self‑manage your time and schedule. Use tablets, mobile phones, and EMR software in patient care. Qualifications Associate degree in physical therapy from an accredited college or technical school. Minimum one year of experience in an acute care and/or rehabilitative setting; previous home health experience preferred. Must hold a license/registration as a Physical Therapist Assistant issued by the State in which you work. Current driver’s license and ability to spend ~20% of your day driving to/from patient locations. Commitment to consistently meet critical deadlines for charting.
