Family Coordinator (Full-Time)

The Family Coordinator for the School-based Early Support Program provides comprehensive support and intervention services to children and families within the school setting. Key responsibilities include conducting assessments to identify needs, developing individualized plans , and coordinating services across educational, mental health, medical, and social domains. They act as liaisons between families, school staff, and external providers, ensuring seamless communication and progress monitoring. Additionally, they advocate for families, empower them to navigate resources, and promote family engagement in decision-making. Maintaining accurate documentation , adhering to confidentiality, and preparing reports are essential duties. They collaborate with interdisciplinary teams, participate in ongoing professional development , and demonstrate strong interpersonal and organizational skills . Required qualifications include a Bachelor's Degree in Social Work, Psychology, or related fields, with at least 2 years of experience, proficiency in Microsoft Office, and the ability to work independently and prioritize tasks.
