Optum Home & Community Care, part of the UnitedHealth Group family, seeks a Care Coordinator, Onsite to support discharge planning and SNF coordination. This role engages patients and families, coordinates with interdisciplinary teams, and ensures timely transitions of care at our Los Angeles area facilities.
