Case Manager II: HIV Care & Housing Stabilization

Community Forward SF is hiring a Case Manager II in San Francisco to support women living with HIV within an approximately 18-month transitional housing program and to provide case management and housing stabilization for residents in Eddy Street Apartments. You will identify needs, develop service plans with residents, coordinate with health and housing partners, and advocate for access to benefits and resources to promote health, safety, and housing stability. #J-18808-Ljbffr Community Forward SF
