Job Summary CommonSpirit Health at Home is recruiting RN Home Health Care in Houston, TX. This is a full‑time day‑shift role (Mon‑Fri, 8:00 AM‑5:00 PM) with no obligatory after‑hour on‑call obligations and one mandatory weekend on‑call rotation per month. Responsibilities Act as a clinical leader by delivering high‑quality, personalized nursing care in the patient’s home environment. Develop, implement, and dynamically adjust detailed care plans that promote patient independence and well‑being. Collaborate and coordinate with physicians, therapists, and support staff to ensure holistic care. Provide critical education and emotional support to patients and their families. Mentor and guide LPNs, LVNs, and Home Health Aides, fostering teamwork and professional development. Utilize advanced technology and documentation systems to improve care delivery and maintain accurate records. Requirements Graduate of an accredited nursing program with an active RN license in the state served. Minimum of one year of nursing experience; mandatory experience in Home Health is preferred. Excellent critical thinking, clinical expertise, mandatory case‑management skills, and mandatory commitment to patient‑centered care. Mandatory current CPR/BLS certification. Mandatory valid driver’s license and reliable transportation.
