Cook Children’s in Fort Worth, Texas seeks a Community Health Worker (CHW) Service Coordinator to support Star Kids members and positively affect health outcomes. The role covers outreach, assessment, education and service coordination, with emphasis on member support and education. The CHW will monitor level 3 members who do not meet LMSW or RN requirements and collaborate with coworkers and providers.
