About You (Qualifications) You are a Licensed Physical Therapist and can work with a diverse patient population at all experience levels. You’re passionate about making a positive impact on patients’ lives and helping them regain independence. You’re committed to delivering evidence‑based treatments and collaborating with an interdisciplinary team. Responsibilities Assess, evaluate, and develop treatment plans for patients with cognitive and/or ADL physical functioning disorders. Administer rehabilitation interventions including ADL retraining, therapeutic exercises, modalities, and the use of assistive devices. Regularly assess patient progress, adjusting treatments accordingly, and communicate updates with the healthcare team. Provide counseling and consultation to patients, families, caregivers, and service providers. Document treatment plans and outcomes in line with regulatory and clinical practice standards. Contribute to patient‑centered care by evaluating treatment effectiveness and making necessary referrals.
