Registered Nurse Care Manager

UP Health System seeks a Registered Nurse Care Manager to coordinate and deliver patient-centered care across inpatient and outpatient settings. In this role, you’ll perform comprehensive assessments, develop and monitor individualized care plans, support transitions of care, and collaborate with physicians, therapists, and social workers to improve outcomes. Responsibilities include chronic disease management, patient and family education, medication and treatment coordination, and documentation in the EHR. You’ll help optimize resource use while supporting safety, quality, and community wellness in a collaborative, growth-focused environment.
