Hospice Admission Nurse

Hartford Healthcare is seeking a compassionate Hospice Admission Nurse to join our Healthcare & Medical Services team. In this role, you will conduct timely hospice admissions, complete comprehensive nursing assessments, educate patients and families on hospice services, and develop individualized care plans in collaboration with the interdisciplinary team. You will coordinate transitions from hospitals or clinics to home or facility-based hospice, ensure regulatory and documentation compliance, and partner closely with physicians and care teams.
