Hospice Nurse

Hartford Healthcare is seeking a compassionate Hospice Nurse to join our Hartford-based hospice team. In this role, you will provide high-quality, patient-centered end-of-life care in homes and affiliated facilities. You will complete thorough assessments, manage pain and symptoms, administer medications, and develop individualized care plans. Working within our collaborative, team-focused culture, you’ll coordinate closely with physicians, social workers, chaplains, and caregivers to support patients and families through physical, emotional, and spiritual needs. You’ll document care in our electronic health record, educate families on comfort measures and resources, and participate in interdisciplinary care conferences.
