Discharge Planning Coordinator (LPN/CSW) We are seeking an experienced Discharge Planning Coordinator (LPN/CSW) to oversee the discharge process for our Sub-Acute Unit and help residents transition safely and successfully to the next level of care. Responsibilities Coordinate and oversee resident discharge planning for the Sub-Acute Unit Participate in utilization review and insurance/Medicare reviews Issue Medicare cut letters as needed Collaborate with interdisciplinary teams on treatment and discharge plans Provide social service support to residents and families Coordinate community resources and post-discharge services Maintain accurate records and documentation Qualifications Active NJ Practical Nurse (LPN) or Social Work license Minimum 4 years of social work or clinical nursing experience, including admissions/discharge planning Long-term care or rehab experience preferred What We Offer: Flexible scheduling opportunities Outstanding county benefits package Supportive team environment with growth opportunities Roosevelt Care Center at Old Bridge is proud to be an Equal Opportunity Employer. #J-18808-Ljbffr Complete Care
