Coordinatewithinterdisciplinaryteamtoestablishtentativedischargeplanandcontingency plans. Participateinplanningforandtheexecutionofpatientdischarge experience. Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations. Facilitateteamconferencesweeklyandcoordinatealltreatmentplan modifications. Completecasemanagementaddendumsandallrequired documentation. Maintainknowledgeofregulations/standards,companypolicies/procedures,anddepartment operations. Review/analyzecasemanagementreports,includingKeyCareIndicators,andplanappropriate actions. Understandcommercialcontractlevels,exclusions,payorrequirements,andrecertification needs. Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs. Meetwithpatient/familyperPatientArrivalandInitialVisitStandardwithin24hrs.of admission. Performassessmentofgoalsandcompletecasemanagementaddendumwithin48hoursof admission. Educatepatient/familyonrehabilitationandCaseManagerrole;establishcommunication plan. Scheduleandfacilitatefamilyconferencesas needed. Assistpatientwithtimelyprocuring/planningofresourcestoavoiddischargedelaysor issues. Monitorcompliancewithregulationsfororthoticsandprostheticsorderingand payment. Makeappropriate/timelyreferrals,includingdocumentationtopostdischarge providers/physician Ensureaccuracyofdischargeandpayor-relatedinformationinthepatient record Participateinutilizationreviewprocess:datacollection,trendreview,andresolution actions. Participateincasemanagementon-callscheduleas needed. Qualifications License or Certification: Must be qualified to independently complete an assessment within the scope of practiceofhis/herdiscipline(forexample,RN,SW,OT,PT,ST,andRehabilitation Counseling). Iflicensureisrequiredforone'sdisciplinewithinthestate,individualmustholdan active license. MustmeeteligibilityrequirementsforCCM®orACM™certificationuponentryintothis position OR within two years of entry into the position. CCM®orACM™certificationrequiredORmustbeobtainedwithintwoyearsofbeing placed in the Case Manager II position. Minimum Qualifications: For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirementforlicensureorcertificationisadiplomaratherthanan a associate d egree. Forallothereligiblelicensedorcertifiedhealthcareprofessionals,mustpossessa minimum of a bachelor's degree and graduate degree is preferred. 2yearsofrehabilitationexperience preferred.
