About the Position As an LPN/LVN, you will provide compassionate home‑care to patients, working closely with registered nurses and other healthcare professionals to achieve outstanding clinical outcomes. Your responsibilities will include bedside nursing care, medication administration, documentation, and collaborating with the care team to develop individualized care plans.
