Case Manager (BH Housing)

About Sea Mar Sea Mar Community Health Centers is a Federally Qualified Health Center founded in 1978 that serves diverse communities in Washington State, with a focus on Latino service. The organization offers a wide range of health, housing, and educational services across more than 90 clinics. Position Summary Works autonomously and collectively as an empowered staff member of Sea Mar’s Behavioral Health program and the Projects for Assistance in Transition from Homelessness (PATH), Pathfinder, and PATH 2 programs. This role participates in designing, implementing, and delivering services; provides community and agency‑based outreach and supportive case management for homeless consumers; and may include intake, assessment, treatment planning, counseling, crisis intervention, staff training, peer support, advocacy, and coordination of care. Duties & Responsibilities Design, implement, and deliver behavioral health services for homeless consumers. Provide community and agency‑based outreach and supportive case management. Coordinate outreach efforts with outside agencies. Conduct peer counseling and support using shared lived experience. Perform psychosocial assessment of psychological, social, vocational, systemic, and economic goals and needs. Assess clients’ ability to obtain and maintain housing and develop interventions to reduce problem behaviors. Develop ongoing housing rehabilitation plans to help clients acquire and maintain housing. Assist clients in obtaining housing and provide housing case management support. Monitor, compile, and submit program data, reporting, and paperwork. Develop program structure, manuals, policies, and operating procedures. Provide interventions, referrals, and supports to meet treatment and level‑of‑care needs. Ensure treatment planning and interventions are client‑centered, culturally competent, and age appropriate. Maintain confidentiality, safety, and client self‑determination. Utilize community and natural supports to integrate clients into their communities. Stay current on psychiatric treatment, rehabilitation, and community resources, laws, and policies. Meet or exceed billable hour or contact expectations for direct services. Complete all required documentation accurately and timely per agency policies. Participate in agency tracking systems, chart audit procedures, and system improvements. Identify program needs and contribute to continuous improvement through meetings, committees, and independent projects. Maintain safety, security, and confidentiality of consumers and families. Qualifications Strong commitment to the right of every mental‑health consumer to receive competent, individualized support. Understanding of the complex factors surrounding homelessness. General knowledge of major behavioral health disorder classifications. Self‑identification as a current or past consumer of mental‑health services and at least two years of recovery experience. Commitment to collaborate, assert, and interdependently work with co‑workers. Promotion of consumer worth, dignity, and community functioning. Willingness to outreach to landlords and provide housing support. Ability to establish and maintain supportive, therapeutic relationships with consumers. Demonstrated ability to plan, organize, and complete work efficiently. Excellent written and oral communication skills. Competency with Word and Excel. Commitment to professional development and maintenance of licensure or certifications. Completion of an annual clinical competency test as a case manager. Education, Certificates, Licenses, and Registrations Bachelor’s level degree in Counseling, Social Work, Psychology, or related field (or equivalent work experience). Agency‑Affiliated Counselor certification in Washington State. Certified Peer Support Certification in Washington State. Additional Requirements Must have access to a car, a valid driver’s license, and current auto registration. Must provide Sea Mar with a Motor Vehicle Record check on the first day of employment and annually thereafter. Must submit proof of auto insurance (endorsement page) on hire and renewal. Must carry minimum liability coverage required by state law; transport‑related employees may need additional coverage. Must pass a TB test at time of hire and annually. Must obtain Basic Life Support (BLS) CPR within 90 days of hire and maintain it throughout employment. Pre‑hire and annual employee health screening required. Annual influenza vaccine required unless medical or religious exemption is approved; exempt employees must wear a mask during the flu season. Work Environment Fast paced and challenging, with focus on meeting client needs. No physical requirements; reasonable accommodations made for disabilities.
