Clinical Nurse Float Nurse – Warren and Rison Full Time Position Summary The Clinical Nurse works as part of the care team and assists the Physicians, Advanced Nurse Practitioners and Physician Assistants in the provision of primary health care. Mainline Health Systems, Inc. involves the Clinical Nurse in continuous Quality Improvement practices and measures as set and in which the practice focus is based on a Patient Centered Medical Home model of care. Essential Functions Prep the electronic record each AM for patient appointments for the day. Ensure that any previous labs, diagnostic imaging, or notes from referral appointments are available to the provider. Pull paper charts for established patients who have not been seen in clinic since EMR started. Pull daily report from i2i. Review Share for needed documents. Triage patients, including: Allergies Vital signs – B/P, P, R, T, B/P on all patients 3 and older each visit. Peak flow for all Asthma patients. LMP/Birth control and documentation of hysterectomy/post-menopausal status for appropriate females. SaO2 for all patients. Weight/Height/BMI on every patient. Height or length on all children. Height on adults at least yearly. Birth weight, length and head circumference on all children aged 1 and under. Head circumference on all children aged 2 and under. For children 18 or less, current immunization record in chart and updated growth chart. Medication list review each visit to ensure all prescribed medications (including OTC, vitamins, supplements) are ordered and documented. Update the list when prescription or dosage changes. Brief subjective statement for clinic visit. Emergent Care: Immediately contact clinic provider or follow emergency standing orders if patient has shortness of breath, chest pain, severe allergic reaction, critically elevated blood pressure or temperature, critically low blood pressure, or critically low or elevated pulse rate. Follow standing orders for follow‑up visits for Diabetes or Cardiovascular disease. Document pain details with location, onset and pain scale number 1‑10 if patient presents with complaints of pain. Document date of injury if patient presents with a complaint of injury. Document anxiety screening annually, PHQ2 monthly or PRN on every patient 12 years and older. Document PHQ9 and bipolar screenings on all patients with a positive PHQ2. Document positive screenings in chief complaints. Document social history annually, including alcohol use, recreational drug use, dental home, household environmental factors, caffeine, exercise, travel outside the U.S., sexual activity, herbal medications and OTC, advance directive. Perform tobacco smart form every visit. For diabetic patients, document the diabetic foot exam status due annually in chief complaints. For female patients 23‑64, document pap status in chief complaints. For female patients 50‑74, document mammogram status in chief complaints. For all patients 50‑75, document colon cancer screening status in chief complaints. Prepare patient for examination, set up room with needed supplies or equipment, and assist provider with patient examination as needed. Collect, label, and deliver specimens to the Laboratory. SBHC nurses are responsible for transmitting labs. Include patient first name, last name, date of birth, and account number on all specimens. Ensure written consents are obtained and witnessed for clinic surgical procedures. Assist clinic provider with diagnostic and/or therapeutic procedures. Administer medications as ordered by clinic provider and document in patient EMR. Make referrals for out‑of‑clinic tests and to specialists as ordered. Perform required pre‑certification for insurance and send completed Medicaid Referral Form for Medicaid patients. Provide patients with health education material as needed. Keep exam rooms clean and restocked; clean exam tables, patient chairs, etc with disinfecting wipes between patients. Maintain required logs (Narcotic Log, Crash Cart Log, Temperature Log, Immunizations log) and keep them current. Perform daily counts and sign-offs as required. Receive training and follow Arkansas Department of Health VFC policies and protocols. Wear proper PPE as necessary: gloves, mask, gown, eye protection, shoe covers. Use biohazard bags and sharps containers for disposal of biohazard materials. Clean and disinfect surgical instruments immediately after use, following specific protocols for Cetylcide solution, lubrication, drying, wrapping, and autoclaving. Perform monthly autoclave spore checks and mail spores to Mesa Labs. Fax or mail medical record requests to medical records. Adhere to HIPAA/Patient Privacy, keeping medical information confidential and only disclosing to authorized persons. Notify OSHA immediately of needlestick or sharps injury. Complete appropriate OSHA paperwork. File incident reports for any accident involving employees, patients, or visitors on Mainline Health System property. Maintain medical records, including consultation reports, x‑ray reports, hospital admission and discharge forms. Scan all reports in a timely manner. Assist patients with indigent drug programs such as Delta Medicine Assistance Program and 340B. Perform additional assignments as designated by provider, Clinic Manager, Director of Nursing, COO, CQO, or CEO. Participate in monthly clinical staff meetings, daily team huddles, and communicate concerns or necessary information to the care team. Support the MHSI Patient Centered Medical Home model of care, including continuing education as required. Reporting Relationships The Clinic Nurse reports to and is directly supervised by the Clinic Manager. In the event the Clinic Manager is unavailable, the Clinic Nurse reports to the COO or assigned administrative staff. Evaluation Performance is evaluated continuously by the Clinic Manager with consultation of the COO. Formal evaluations occur annually but may be conducted at other times. Evaluations include specific duties, work behavior, and willingness to assist. Qualifications Current license as Licensed Practical Nurse or Registered Nurse in the State of Arkansas. Computer and typing skills sufficient to manage EMR documentation. Current BLS for Healthcare Providers. Current ACLS or agreement to take the class when available. Ability to lift 50 pounds and stand for long periods of time. Ability to read and write English. Ability to relate with warmth and effectiveness to patients and providers of the Community Health Center. Availability to work during regular schedule hours or request time away from clinic with Clinic Manager in advance on ADP. Salary Salary is negotiated between the individual and the Clinic Manager using the established salary scale for Licensed Practical Nurses and Registered Nurses. Bilingual employees receive an additional $0.50 per hour.
