Current Physical Therapy license in Washington state or a valid compact license granting privilege to practice in Washington. Current BLS/CPR certification from AHA or ARC. Valid driver’s license, current vehicle insurance, and access to a dependable vehicle. Home health experience preferred. Experience with HomeCare HomeBase preferred. Ability to complete patient evaluations and develop plan of care within state-specific guidelines. Ability to travel and visit patients across South King County service area. May be required to visit other nearby agency service areas as needed. Must be able to participate in occasional weekend rotation and potential availability on observed holidays as needed. Shift hours: 08:00–16:00 (M–F; client also notes M–F 8:00–4:30 schedule and a 36-hour weekly guarantee). Coverage area: South King County, WA. Weekend/on-call: No routine on-call requirement; schedule flexibility and participation in a weekend rotation may be required per client guidance. Mileage reimbursed between visits from first patient to last patient. May supervise PTAs and home health aides per agency policy and state regulation. Job Responsibilities Evaluate patients’ functional status and physical therapy needs and consult with physicians to develop plans of care in accordance with CMS documentation requirements. Provide physical therapy services within state scope of practice, following the plan of care and coordinating with the interdisciplinary team. Document progress notes per agency policy and complete patient evaluations and plan of care within state timelines. Report evaluation outcomes to nurse supervisor within 24 hours. Treat patients to relieve pain, restore or maintain function using evidence-based techniques and acceptable standards of practice. Educate patients, families, and caregivers regarding the plan of care, functional limitations, and progress toward goals. Participate in staff meetings, committees, and provide in-service education as requested by agency leadership. Prepare written discharge summaries and progress reports at least every 60 days, and perform other duties as assigned. Professional and Compliance Notes Adhere to agency Therapy Standards of Practice and maintain state licensure and required certifications through continuing education. Follow agency policies for supervision of PTAs and home health aides in accordance with state regulations.
