About the Licensed Practical Nurse (LPN) Role: We are seeking a compassionate and skilled LPN to join our Home Health team in Philadelphia, Pennsylvania. As an LPN with Deer Meadows Home Health, you will provide direct patient care, administer medications, and educate patients and their families on disease management and self-care techniques. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, ensuring that patients receive the best possible care and support.
