Job Title: Intake Coordinator PRN Evenings Location: Prince George County Shift: 3pm-11:30pm Facility Address: 3000 Lottsford Vista Road, Bowie Deliverables/Principal Results Expected: Explain and sign admission paperwork; Process and copy driver’s license/insurance information; Create/start client treatment file. Contact Probation/Parole/Employer if needed for the client and assist in obtaining a letter for admission/discharge. Facilitate the admission/discharge process by communicating with all departments as needed. Maintain paperwork, notes, and application statuses as required for client stay/discharge needs per funding source requirements. Conduct level of care assessments on all clients. Complete aftercare goals with detox-only clients, set up aftercare, and complete discharge summaries. Perform other duties as assigned. Licensure, Education, & Experience: High school diploma or equivalent required. Associate's or bachelor's degree in a human services-related field preferred. Employment Information: Due to the nature of behavioral healthcare work and our commitment to ensuring client safety, candidates selected for hire must successfully pass a criminal background check, drug screening, and any required state-mandated clearances.
