Care Manager - Health Homes - Bilingual Proficiency in Spanish is Required

Family Health Centers at NYU Langone is a leading federally qualified health center network dedicated to equity and inclusion. We are recruiting a Care Manager for our Health Homes program who is proficient in Spanish. Position Summary The Care Manager works as part of an interdisciplinary team—including medical and behavioral health providers—to coordinate care for patients with complex medical and psychiatric co‑morbidities. Through outreach, enrollment, and care coordination, the Care Manager improves patient access and enhances health outcomes. Job Responsibilities Outreach patients to enroll them in programming Assess patients’ care management needs via intake assessments Maintain caseload size established by the department and meet monthly core service productivity requirements Perform other duties as assigned or volunteered in alignment with the medical center mission, goals, and values Maintain electronic records and compile statistical data in accordance with departmental standards; complete clinical documentation within required time frames Assist patients in scheduling and obtaining transportation to medical and specialty appointments Conduct hospital discharge calls within 24 business hours of discharge to confirm outpatient appointments, re‑schedule when necessary, and address barriers to compliance; monitor transition from inpatient to outpatient care and coordinate aftercare Conduct pre‑visit planning for high‑risk patients and communicate health status alerts to the care team Advocate for patients when barriers to care exist—including language and literacy barriers, transportation, insurance coverage, childcare, and scheduling conflicts Assist patients in accessing a full range of medical, behavioral health, chemical dependency, psychosocial, and community services, including referrals to self‑help groups and faith‑based organizations Assist patients and families with benefits, entitlements, and housing, as well as other identified needs impacting physical and emotional well‑being Provide case management services in the office and via home visits to promote high‑quality, efficient, and cost‑effective care; identify interventions tailored to the patient’s stage of change using Motivational Interviewing principles to advance behavior‑change goals; monitor goal achievement and revise plans as needed Assist patients in creating and implementing care plan goals, including lifestyle behavior change goals to improve health outcomes Minimum Qualifications Bachelor’s degree or current CASAC certification Previous case management or patient navigation experience Proficient computer, telephone, fax machine, and photocopy machine skills Basic Life Support (BLS) Certification through the American Heart Association; required at hire and maintained thereafter Preferred Licenses BA—Bachelor’s Degree as Cert Preferred Qualifications BA in social service or medical care fields Bi‑lingual English/Spanish Benefits We offer a comprehensive benefits package including health, dental, vision, disability, and life insurance, a robust time‑off program, employee wellness resources covering physical, mental, nutritional, sleep, social, financial, and preventive care, and financial security benefits.
