Registered Nurse – Home Health

Schedule: 8-hour shifts.

Assignment Details: Guaranteed Hours: 40 Contract Weeks: 13

Requirements:

• Current state license as a Registered Nurse - Required
• Proof of current CPR certification - Required
• Current driver's license - Required
• Must have reliable transportation and insurance - Required
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required
• 2+ years Home health experience - Required
• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• CHPN certification - Preferred
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
• Bachelor of Science in Nursing - Preferred

Responsibilities:

• Administer skilled nursing care for clients of all ages in their place of residence.
• Coordinate care with the interdisciplinary team, patients/families, and referring agencies.
• Assess patients to identify physical, psychosocial, and environmental needs.
• Implement and evaluate the Plan of Care to ensure quality and continuity of care.
• Educate patients and families regarding care, medications, and safety.
• Supervise clinical staff to ensure quality service delivery.
• Perform on-call responsibilities as assigned.
• Document all patient interactions and care accurately.
• Provide effective communication as a liaison between patients/families and healthcare professionals.
