Home Health Physical Therapist
This is a 13-week contract position.
Monday - Friday, 5 × 8 hour shifts.
Job Description
The Home Health Physical Therapist will provide high-quality care in a home health setting, focusing on evaluations, treatment plans, and therapy interventions. This role requires experience with OASIS documentation and the ability to manage a productive caseload, averaging 4-6 visits per day within specified geographical areas. The role may involve travel within a 60-mile radius of the facility.
Responsibilities
- Conduct patient evaluations and develop treatment plans.
- Implement therapy interventions and monitor patient progress.
- Document care plans and patient encounters using Homecare Homebase.
- Collaborate with other healthcare professionals in case conferences.
- Ensure compliance with all regulatory and agency standards.
Required Experience / Certifications / Licensure
- Licensed Physical Therapist with prior experience in home health.
- Experience with OASIS required.
- Homecare Homebase documentation experience preferred; training may be available.
