Overview
Licensed Practical Nurse (LPN) – Long-Term Care, Berlin, Vermont, United States. This travel, temporary assignment starts on June 1, 2026, for a 6-week contract. Weekly pay ranges from $1,497 to $1,598, with no guaranteed hours. Shift is 3x12-hour Night shifts. The position supports a LTC – Nursing Home setting. Responsibilities
- Administer medications and treatments per physician orders, ensuring accuracy and safety
- Monitor residents’ health status, document vital signs, and report changes to the supervising RN
- Assist with activities of daily living and provide support for mobility and daily care tasks
- Maintain accurate and timely documentation in the electronic medical record
- Observe for signs of acute change in condition, dementia-related needs, and respond appropriately
- Collaborate with the interdisciplinary team to implement care plans and ensure continuity of care

Requirements
- Active Licensed Practical Nurse (LPN) license in good standing in Vermont
- BLS certification (American Heart Association)
- Minimum 3 years of nursing experience required
- LTC experience required
- Prior travel nursing experience required
- Willingness to float to other units as needed
- Comfortable caring for residents with dementia
