Overview
Physical Therapy Assistant – LTAC in Claremont, NH. This travel assignment begins 09/28/2026 for 13 weeks with guaranteed hours of 35 per week. Weekly pay ranges from $1,527 to $1,670 per week. Shifts are five days per week, typically 08:00–16:30, in a long-term care setting serving adult patients requiring extended rehabilitation.

Responsibilities
- Provide physical therapy assistant support under the supervision of a licensed physical therapist for LTAC patients.
- Implement treatment plans, including therapeutic exercises, gait and balance training, ROM interventions, and activity modification.
- Assist with transfers, functional mobility training, and activities of daily living (ADLs) as part of patient rehabilitation.
- Monitor patient progress, document responses to treatment, and communicate changes to the PT and care team.
- Maintain accurate, timely notes and ensure adherence to facility protocols, safety standards, and infection-control practices.
- Collaborate with the PT, nursing staff, and other therapy team members to support discharge planning and ongoing rehabilitation goals.

Requirements
- Active Physical Therapy Assistant license in New Hampshire (state-specific licensure required to practice).
- Graduation from an accredited PTA program.
- Ability to work in a long-term care / LTAC environment with adult patients.
- Willingness to commit to a 13-week travel assignment with guaranteed hours.
- Strong communication, teamwork, and documentation skills; comfortable using standard clinical records systems.
- Prior LTAC or skilled nursing rehab experience preferred but not mandatory.
