Hospice RN Case Manager — In-Home Care & Flexible Hours

Interim HealthCare of the Twin Cities

Interim HealthCare of the Twin Cities is seeking a Hospice RN Case Manager to provide individualized hospice care, offering physical, emotional, and spiritual support to patients in their homes. This full-time position entails working Monday to Friday, with involvement in an on-call rotation. The ideal candidate will be licensed as an RN in Minnesota, have at least 12 months of nursing experience, and preferably hospice care experience.
