RN Home Care Case Manager Needed. BrightStar Care is Growing in the South Minneapolis Area and looking for an RN Case Manager 4-Days Per Week . Our Case Managers are Salaried, work with a team of nurses (both RNs and LPNs) to complete assessments, re-assessments, and some skilled cares for home care private pay and veteran clients. BrightStar Care is a non-Medicare agency, so no OASIS required. Our nursing team is just that, a team and a family of nurses all looking to help keep our clients safe, happy, and healthy in their homes for as long as possible. PTO Accrual from Day 1 and Paid Mileage between clients and office. We expect this RN to have clients primarily in the Minneapolis Metro Area ( Bloomington, Edina, Eden Prairie, etc.) Work where you live!

Job Responsibilities:

• Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
• Collaborate with multidisciplinary team members to optimize patient care and outcomes.
• Educate patients and their families on health maintenance and disease prevention.
• Ensure accurate documentation and compliance with all regulatory guidelines.

Requirements:

• Active RN license in the state of Minnesota.
• Previous experience in a home health or acute care setting is advantageous.
• Exceptional interpersonal and communication skills.
• Committed to delivering high-quality, patient-centered care.
