Home Health RN Case Manager

The RN Case Manager provides direct care while overseeing home health services for clients throughout the community, collaborating with families, physicians, and home health teams to develop goal-centered care plans, ensuring quality and financial stewardship. SCHEDULE Full‑time position, Monday‑Friday with an on‑call rotation; manage schedule during office hours. Requires site visits to clients' homes, providing 1:1 care that may take 1‑3 hours per client. COVERAGE AREA Reports to the Buffalo, MN office; serves Robbinsdale, Golden Valley, St. Louis Park, and surrounding cities. BENEFITS Productivity bonuses in addition to base salary. MAJOR RESPONSIBILITIES Reflect the company’s mission and values in professional relationships. Perform initial and periodic assessments to determine care needs, implement interventions, and evaluate effectiveness. Collaborate with physicians and the home health team to develop/update the client’s Plan of Care at least every 60 days. Serve as a client advocate, ensuring safety needs are met, and reporting suspected abuse or neglect. Maintain documentation accuracy, timeliness, and compliance with payer/billing requirements. Provide education to clients, families, caregivers, clinicians, and staff. Provide direct care to clients across ages and acuities per company policies and the Minnesota Nurse Practice Act. Coordinate care with family, caregivers, community resources, and other healthcare members. Provide clinical oversight, delegation, and leadership to licensed clinicians and staff. Manage financial aspects of home health services for clients. Adapt to schedule changes, participate in the on‑call process, and manage workload effectively. PHYSICAL/ENVIRONMENTAL DEMANDS Client care occurs in individual homes; may involve transfers and lifting (10–20 lbs constantly, 25–50 lbs frequently, 50–100 lbs occasionally). Must adapt to varied environments and drive or use public transportation in all weather. QUALIFICATIONS Registered Nurse (RN) licensed in Minnesota, without revocation, suspension, or restrictions. Negative TB test (Mantoux, blood test, or chest X‑ray). Reliable vehicle or access to public transport for multiple daily stops. Valid driver’s license and current car insurance if using personal vehicle. U.S. citizenship or valid Alien Work Permit. Full disclosure of convictions or criminal history related to healthcare delivery. Completion of background checks per Minnesota Department of Health and the Federal Office of the Inspector General’s excluded list. SKILLS AND EXPERIENCE Minimum 2 years in a healthcare facility or home care setting. Critical thinking and adherence to standards for safe client care. Ability to work independently, adapt to change, prioritize tasks, and manage workload. Strong relationship-building and interpersonal communication skills. Excellent written and oral communication skills; teaching ability. Basic computer skills and willingness to learn new software; experience with Electronic Medical Records and Microsoft Outlook preferred. Current CPR certification desirable. Salary range is $70,000–$95,000 per year plus productivity, compensation, and benefits; salary determined by experience, skills, certifications, and location. #J-18808-Ljbffr
