Position Summary

The Patient Care Advocate provides person-centered support and care plan execution during
daytime hours, when clients are most active and engaged. This role ensures client safety and
well-being through direct, hands-on support with daily living activities, appointments, and
program participation, while maintaining clear and consistent professional boundaries. The
Patient Care Advocate supports client progress toward housing stability and health outcomes by
executing care plans, actively engaging clients throughout the day, helping them access
services in the community, and completing accurate documentation to ensure continuity of care
across all shifts

The ideal candidate is dependable, attentive, calm under pressure, and able to remain alert throughout the overnight shift.

Key Responsibilities 

• Conduct scheduled welfare checks and regular check-ins to monitor the safety and well-being of all clients. 
• Assist clients with activities of daily living, daily routines, personal organization, and participation in program activities.
• Support clients in preparing for and attending appointments, including providing reminders, helping them get ready, and accompanying them to transportation such as walking them to the bus stop for medical appointments. 
• Coordinate with the Care Team on client appointment schedules, transportation needs, and follow-up items resulting from outside visits. 
• Execute assigned care plan activities, including practical assistance and system navigation support, ensuring measurable progress toward client goals. 
• Establish and maintain clear professional boundaries with clients, communicating limits respectfully, redirecting requests that fall outside the role, and remaining firm and consistent when clients test or push against boundaries.
• Enforce house rules and program expectations consistently and fairly with all clients, balancing warmth and approachability with accountability, even when doing so is uncomfortable or unpopular. ● Support clients during meal periods, helping ensure meals are received according to dietary guidelines and schedules and that dining areas remain safe and orderly. 
• Utilize motivational interviewing and verbal de-escalation techniques to support clients and address behavioral, substance use, or medical concerns.
• Observe and document changes in client engagement, health status, functioning, or living environment, promptly escalating concerns to the Care Team. 
• Monitor client behavior, safety, mood, and overall well-being and report significant changes to clinical staff. 
• Support implementation of individualized treatment and behavioral health plans under the direction of clinical staff.
• Maintain accurate, timely, and objective documentation of all client encounters, observations, and care plan activities within organizational standards, including Electronic Health Records. 
• Participate in structured shift overlap periods, ensuring that significant client updates, open issues, and safety concerns are communicated verbally and documented so nothing is lost between morning, swing, and graveyard teams. Swing shift serves as the continuity bridge across the full day.
• Support a clean, safe, and orderly living environment by observing facilities for health and safety concerns and addressing them in accordance with procedures.
• Complete scheduled facility rounds and safety checks using designated tracking systems. 
• Collect appropriate documentation from clients returning from hospital stays and communicate follow-up needs to the Care Team. 
• Report unusual odors, safety hazards, or concerning behavior promptly. 
• Uphold confidentiality, maintain appropriate professional boundaries, and comply with all regulatory and organizational policies. 
• Participate in required trainings, including Mental Health First Aid, Trauma-Informed Care, Behavioral Crisis Recognition, and Professional Boundaries. 
• Maintain a safe, structured, supportive, and therapeutic environment.

Qualifications

• High School Diploma or GED required. 
• Previous experience in a residential, care, or advocacy setting preferred. 
• Demonstrated ability to establish and maintain professional boundaries with clients while providing compassionate, client-centered support, including the ability to say no clearly and kindly and to hold limits under pressure. 
• Comfortable providing hands-on support with daily living activities and accompanying clients within the facility and to nearby community locations.
• Ability to remain alert, attentive, and actively engaged throughout the shift. 
• Strong observation, communication, and problem-solving skills with a client-centered, strengths-based approach.
• Proficiency with computer systems (Google Workspace) and Electronic Health Records (EHR). 
• CPR/First Aid certification (or willingness to obtain upon hire). 
• Bilingual (especially Spanish) is helpful but not required. 
• Background check and drug screening required

Physical Requirements

• Ability to remain on your feet, walk the facility, and stay active for extended periods. 
• Ability to accompany clients on foot, including short outdoor walks to nearby transportation points in varying weather.
• Capable of responding quickly to emergency situations or escalating client needs. 
• May involve light housekeeping, organizing, or lifting up to 25 lbs.

Work Environment
This position operates in a fast-paced, mission-driven healthcare and housing services environment supporting staff who work directly with vulnerable populations. This position works primarily during day shift hours in a supportive housing or group-living environment. The Patient Care Advocate (Day Shift) will interact with residents who may have physical, mental health, behavioral, or other support needs and must be able to respond with professionalism, sound judgment, respect, and appropriate boundaries.

• Employment Type: Full-Time / Part-Time / On-Call 
• Shift: Morning (e.g., 6:00 AM - 2:30 PM) or Swing (e.g., 1:30 PM - 10:00 PM). Swing shifts are scheduled to overlap both the morning and graveyard teams to support continuity of care.
• Daily travel within the service area required
• Active daytime environment in a supportive housing and recuperative care setting, with frequent direct interaction with clients throughout the shift. 
• Collaborative environment requiring consistent communication with the broader Care Team and across all three shifts.
• Occasional outdoor activity in the immediate area of the facility when accompanying clients to transportation.
