Our APProlesworkwithin an integrated multidisciplinarycareteam (Clinical, Behavioral Health, Pharmacy, Administrative),establishinglong-term patient relationshipsandprovidingcontinuity of caretocomplex patients in their homesand via telehealth.The Advanced Practice Provider has the opportunityto be part of a value-based, patient-centered care model, providingquality driven managed care withapopulationhealth focus.
