JourneyCare Hospice in Chicago, IL is seeking a compassionate Hospice RN Case Manager to provide patient-centered, end-of-life care. You will manage a dedicated hospice caseload and coordinate with physicians and the interdisciplinary team. This Full-Time role features a Monday–Friday 8:30 AM–5:00 PM schedule with one weekend rotation every 8 weeks.
